Guidelines and statistics relating to prescribing in diabetes
There has been a lot of discussion regarding the Insulin Passports, as you know this consists of the credit card fold out leaflet and the Insulin safety booklet. Both need to be given together and can be purchased from NHS Supplies using the code: WRU2703 (for the passport) and WRU2521 (for the leaflet). Please note that the Community Diabetes Specialist Nurse Service cannot provide you with these.
Diabetes information prescriptions
An electronic version of a personalised Diabetes UK Management Plan is available for completion in EMIS Web and InPS Vision IT systems. The plan can be used to record the objectives and steps agreed with the patient and generate an Information Prescription.
The Information Prescription can be printed out and given to the patient and an electronic version saved in the patient’s care record.
The Diabetes Management Plan alert needs to be activated and will generate a pop up alert for patients with a raised BP, raised HbA1c or raised Cholesterol value and no record of a diabetes management plan in the last six months.
In Vision the information prescriptions will be immediately available, but they have to be activated in EMIS. Instructions for installing in EMIS are available here.
