Rapid Response and Treatment Service

The Rapid Response and Treatment Service is a medically led multidisciplinary service whose aim is to support you to keep your patients/residents who experiences a sudden illness or deterioration of a chronic illness in their care home. 

Following assessment the team will agree that your resident’s condition is appropriate to be managed in the care home under their care with the support of the GP The team will discuss your resident’s needs with you and set up a personalised programme of treatment, support and nursing care which will be delivered in your care home.

The aim of the service is to assist people to remain in their care home with the right support to meet their needs, and avoid hospital admission.

Who can I refer to the service?
You can refer residents with a new episode or sudden worsening of their condition, such as the following:

  • Confusion
  • Fluid retention
  • Breathlessness
  • Difficulty passing urine
  • High temperature
  • Cellulitis
  • Symptoms of chest infections, COPD and asthma
  • Reduced mobility
  • Diarrhoea and/or vomiting
  • Falls
  • Dehydration
How do I refer a resident to the service?

The service is available from 9am to 7pm 7 days a week, 365 days a year. Referrals can be made through the health hub at any time but will need to have been accepted by the service before 5pm to receive a same day assessment. What types of treatments are residents offered?
  • Short-term IV therapy/fluids
  • Medication review and changes
  • Nebulisers
  • Functional assessment / management
What to expect from rapid response and treatment?
  • A nurse will agree an appropriate schedule of visits to support each patient on an individual basis and visit accordingly and will arrange for the doctor to visit the resident if this is appropriate. The nurses caring for patients on this short term pathway are available from 9am to 7pm, 7 days a week.
  • You will continue to provide medicines support for your residents as normal during the patients period on the pathway. Your residents care plan may include giving additional medicines or fluids intravenously (directly into the bloodstream) and the nurses will provide support for this more complex administration.
  • All of this can be provided at the care home through the service.
How long will this service last?

Rapid response and treatment is provided for a maximum of 5 days, but many people do not need our services for this long. During this time we will review your resident’s situation and adjust support accordingly.
